Radical methods of treating coronary heart disease are aimed at directly increasing coronary blood flow. For this purpose, percutaneous transluminal coronary angioplasty is increasingly performed. Indications for the Procedure

Percutaneous angioplasty is aimed at dilating vessels whose lumen has narrowed due to atherosclerosis – cholesterol deposits on the inner walls of arteries.

The procedure is prescribed to patients with:

  • severe angina pectoris, including variant angina;
  • previous myocardial infarction;
  • pre-infarction state;
  • proven ischemic disease;
  • multiple heart vessel lesions.

Minimally invasive angioplasty surgery is indicated for patients who are not helped by drug therapy for angina and ischemia. The decision on stenting is made only based on the results of coronary angiography – an X-ray contrast method for examining heart vessels.

How is the Procedure Performed?

A balloon catheter is used to dilate narrowed or blocked vessels. This is a medical device that can inflate to fixed sizes. Artificial pressure inflation forces the muscular walls of the arteries to expand, thereby ensuring normal blood flow.

Tissues that are supplied by the pathological vessel begin to receive more nutrients and oxygen. As a result, pain syndrome disappears, angina attacks stop, and the patient's general well-being improves.

Percutaneous transluminal coronary angioplasty is performed according to the following algorithm:

  1. The doctor gains access to the patient's vascular system through the femoral or radial artery. A large surgical incision is not required, a small puncture is sufficient.
  2. Through the puncture and installed introducer (dilator tube), a guide catheter is inserted, reaching the area with the damaged coronary vessel.
  3. One coronary balloon is guided to the zone of maximum narrowing.
  4. In several stages, the balloon is inflated, gradually expanding the artery and facilitating blood flow.

PTCA is often combined with stenting. The intervention is carried out according to the same scheme, only a stent is attached to the surface of the balloon in a folded state. At the moment of inflation, it unfolds and is tightly pressed against the walls of the vessel. The stent remains inside the artery, performing the function of a frame and preventing it from narrowing again.

Transluminal angioplasty is most often performed under local anesthesia and takes no more than 2 hours. Stenting always ends with cardiography – the doctor needs to make sure that blood flow is restored and there are no blood clots. After PTCA, the patient spends a day in cardiac intensive care and several days in a ward of the medical center.
