Carotid Artery Stenting in Germany

Carotid arteries – are the main vessels that supply blood to the brain. Carotid artery stenting in Germany allows restoring full blood flow in this part of the body in case of atherosclerotic lesions and thus preventing ischemic stroke.

What is Artery Stenting?

Stenting, or angioplasty – is a minimally invasive operation that involves installing a thin wire structure (stent) into the lumen of a vessel. The stent performs a scaffold function – it returns the vessel to its normal state, preventing it from narrowing again.

Stenting is indicated for:

  • vessel narrowing of 70% or more;
  • signs of stroke and mini-stroke;
  • stenosis (narrowing) in a location difficult to access for endarterectomy.

The procedure is also prescribed for patients with contraindications to open vascular surgeries.

Surgical Treatment Methods

Stenosis of the carotid and brachiocephalic arteries is most often formed as a result of atherosclerosis. Cholesterol plaques form on the inner walls of blood vessels, which over time grow, completely or partially blocking blood flow.

German clinics use 2 methods to combat this condition:

  • carotid endarterectomy – an open operation aimed at removing the inner wall of the carotid artery;
  • stenting – installation of a mesh stent that keeps the walls in an open state.

Stent implantation is the preferred method of therapy for atherosclerosis, since it is performed minimally invasively and rarely causes complications.

Preparation for Surgery

The procedure is preceded by comprehensive diagnostics. It allows assessing the degree of vascular damage, the characteristics of cerebral circulation and finding out the localization of the atherosclerotic plaque. Diagnostic activities include:

  • duplex ultrasound scanning;
  • angiography;
  • computed tomography;
  • magnetic resonance angiography.

A week before the operation, the patient should take aspirin – a drug that reduces blood clotting.

Course of the Operation

The surgical intervention is performed under local anesthesia, but in some cases general anesthesia is indicated. The patient is connected to digital equipment that monitors vital signs (pulse, blood pressure, breathing). The medical procedure in Germany takes place in several stages:

  1. Through the femoral artery, a sheath with a mini-balloon at the end is inserted into the lumen of the vessel.
  2. Angiography is performed again to determine the exact localization of the stenosis.
  3. Using the balloon, the lumen of the carotid artery is expanded.
  4. A guidewire and filter-trap are brought through the vascular bed to the site of narrowing, which prevents the development of thromboembolism.
  5. Using another catheter, a stent is inserted into the problem area, carefully positioned and secured.
  6. The filter-trap and catheter are removed, and the stent remains in place.

The operation takes about 2 hours, while the patient remains conscious but does not feel pain.

Postoperative Period

After the procedure, to prevent bleeding, the patient must adhere to bed rest for several hours. In the early postoperative period, taking baths is prohibited (only showers are allowed) and lifting heavy objects. A special drinking regimen is indicated – it is necessary to drink as much fluid as possible to remove the contrast agent and anesthesia drugs from the body.

At first, the catheter insertion site may be painful and swollen – this is a normal reaction to the intervention. At this stage, the doctor may prescribe medications that reduce discomfort and thin the blood. In the postoperative period, German clinics perform repeat angiography to monitor the stent location and the condition of the carotid artery.

Contraindications for Carotid Artery Stenting

Risk factors include:

  • heart rhythm disorders;
  • allergic reactions to drugs used during the procedure;
  • hemorrhagic strokes in the last 2 months;
  • complete vessel blockage;
  • high blood pressure;
  • stenosis of upper and lower extremity vessels;
  • presence of large plaques at the origin of the carotid artery.
